Member/Unit Activity Forms

To assist Division members in completing mission reports, the SO-IS has pre-filled the National forms to include Division 22 and the SO-IS email address in the "Recipient" field. You can complete the forms online and the SO-IS will automatically get a copy for entry into AUXDATA.

Security Forms

Direct Operations. If you are an elected officer (RCO or higher), on the National staff, or otherwise required to have a higher level of security clearance (Fingerprint Technicians), you must complete the SF86 Questionaire for National Security.

New Member Forms

These forms are required for membership. In addition to the forms below, we will take three sets of fingerprints and a photograph. For your photo, please wear a light blue or white collar, button down shirt. You will need to sit for a New Member Exam (Open book).
